SHM considers it very important to treat your data in a careful and secure manner. This privacy statement explains to you which data is collected, for what purposes this data is used and how SHM handle your data.
General principles
SHM applies a number of core values:
- Provision of information; We aim to inform you as thoroughly as possible about how and why we process personal data. We do so by means of this privacy statement and extensive information we share on our website.
- Restriction of the collection of personal data; We take utmost care to ensure that the collection of personal data remains restricted to only that which is necessary for the purpose for which the data is collected.
- Protection of personal data; We take measures (both technical and organisational) to protect your data and require third parties that process personal data on our instruction to do the same.
- Consent for processing of personal data; We request consent for processing your personal data in those cases where this is required under the General Data Protection Regulation.
- Rights of data subject; We respect your rights with regard to the processing of personal data.

Processing of your personal data
We process personal data for different purposes. Below, we explain which personal data we process, how the personal data is used and how long this data is stored.
HIV monitoring and research
SHM has been appointed by the Minister of Health as the implementing agency for monitoring people with hiv in the Netherlands. SHM registers a number of basic data for this monitoring:
- year of birth;
- sex;
- born in the Netherlands yes/no;
- date in care and out of care;
- data from an earlier registration with SHM, if you have previously attended another HIV treatment centre;
- statement that written and/or oral explanation has been given.
If you have gave us your permission to participate in our scientific research, we register the following data:
- date of birth;
- initials (max. 2 letters);
- gender;
- nationality and country of birth;the 4 numbers of your postcode (no letters);
- date in care and out of care;
- data from an earlier registration with SHM, if you have previously attended another HIV treatment centre;

hospital registration number;statement that written and/or oral explanation has been given.
We need this information to make collection of your medical data possible. You will find detailed information about why we collect data, what data we collect and how you can withdraw your consent on our website under the tab "What we do". Your data will be stored for as long as this is necessary for the research.
When you withdraw your consent, your registration data will be deleted and your basic data will be registed.
Contactdetails HIV treatment centres
To be able to share the results of research, the following data is collected of the treatment teams:
- name;
- gender;
- jobtitle;
- e-mail (work).
Known changes are processed immediately and personal data is checked and deleted once a year if the person concerned is no longer working for the treatment team.
Visiting SHM
If you visit SHM, camera images are made by the Weesper for the safety of all persons who visit the building. These camera images are destroyed within 4 weeks and only kept longer if necessary for dealing with an incident by the police.
Website Usage
To manage your cookie preferences, SHM uses a consent management platform. When you set your cookie preferences, the following information is processed:
- whether you have given or refused consent;
- the cookie categories you have selected
- the date and time of your choice;
- a unique consent identifier (consent ID);
- limited technical information, such as (part of) your IP address and information about your browser or device, where necessary to record your choice and demonstrate that valid consent has been obtained.
Your cookie preferences are stored in a cookie on your device for a maximum of 12 months, unless you delete the cookie earlier or change your preferences.
Records of your cookie consent are retained for a maximum of 5 years to demonstrate that valid consent has been obtained, where required by applicable privacy legislation. After this period, the records are deleted or anonymised.

If you have any questions or comments about SHM, you can contact us directly via the website. Simply complete the contact form on the website.
If you use the contact form, we require certain personal data from you to enable us to handle your request. You will therefore be requested to provide the following data in the contact form:
- name
- organistation (not obligatory)
- your question or request
The personal data that you provide through the contact form will be stored by us for as long as is necessary to handle your question in an appropriate manner. We will remove your personal data as soon as it is clear that your question has been answered to your satisfaction. Our staff may ask you to provide additional data to be able to handle your question.
Job application
You can submit your job application through our website. We will request you to e-mail your motivation letter and Curriculum Vitae. We will store the personal data that you provide for the duration of the job application procedure and for a period of one months afterwards. With your consent we may store your personal data for a longer period of time, but for no longer than six months.
Publications
SHM collaborates with research institutes in various projects. As a result, the names of researchers are mentioned on our website.
Third party websites
This statement does not apply to third-party websites to which our own website refers. We cannot guarantee that these third-parties will handle your personal data in a reliable or secure manner. We therefore recommend that you read the privacy statements relating to such websites before using them.
Security
SHM considers it of utmost importance to treat your personal data in a secure manner. Therefore we have taken technical and organizational measures to protect your data against loss or unauthorized use.
- SHM uses the infrastructure and servers of Amsterdam UMC. Amsterdam UMC is ISO 27001 and NEN7510 certified and the servers are located at Amsterdam UMC locations and managed by Amsterdam UMC employees.
- Our website is secure, personal data you provide us through the website is sent encrypted.
- SHM has an internal policy that safeguards careful handling of your personal data.
Your rights
Under the General Data Protection Regulation, you have a number of rights concerning your personal data:
- Right to erase/be forgotten; You have the right to have your data removed by withdrawing your previous given permission.
- Right to object; You have the right to object if you think that your data will be used for something other than the original purpose.
- Right to rectification; You have the right to have your personal data, that are incorrect or incomplete, corrected.
- Right to restrict processing; You have the right to (temporarily) stop the processing of your personal data.
- Right to access; You have the right to view your (personal) data.
- Right to data portability; You have the right to transmit your data to another party.
You can contact SHM if you want to exercise your rights, or if you have a question about the way in which SHM handles your personal data. To prevent misuse, SHM may ask you to identify yourself if you contact us to exercise your rights.
Dutch Data Protection Authority
If you have any complaints about how SHM processed your complaint, you have the right to submit a complaint to the Dutch Data Protection Authority (the Dutch supervisory authority in the field of privacy) or another European supervisory authority in the field of privacy.
